NSFAS Vulnerable Child Declaration Form The Vulnerable Child Declaration Form is an essential element of the NSFAS application process, meant to identify children who may be vulnerable due to unique circumstances. The Vulnerable Child Declaration Form can be downloaded directly from the NSFAS website.

It’s a single-page document requiring completion by a registered social worker from the Department of Social Development (DSD).
