What does vulnerable child mean? This form is only to be completed by applicants who are under the age of 18 and who are orphans OR, who are vulnerable children in that they have been exposed to abuse, neglect, exploitation, risk of violence and substance abuse.

Who Needs the Vulnerable Child Declaration Form?

This form is required for students who qualify as vulnerable children but do not receive financial support from SASSA.

This could be because:

  1. The student is newly orphaned and hasn’t yet applied for SASSA support.
  2. The student’s circumstances don’t meet the exact criteria for SASSA grants.

How to Obtain the Vulnerable Child Declaration Form

The Vulnerable Child Declaration Form can be downloaded directly from the NSFAS website.

It’s a single-page document requiring completion by a registered social worker from the Department of Social Development (DSD).

What Information Does the Form Require?

The social worker will fill out the form, including the following details:

  1. The social worker’s name, position, and DSD office
  2. The student’s full name and ID number
  3. Confirmation that the student qualifies as a vulnerable child based on the SASSA definition

Who Can Complete the Vulnerable Child Declaration Form?

Only registered social workers employed by the DSD can complete this form.

They will have access to the student’s case file and can verify their vulnerability status.

What documents do I need to submit with the Vulnerable Child Declaration Form?

In addition to the completed form, you may be required to submit supporting documentation depending on your specific circumstances.

This could include:

  1. Death certificates of deceased parents (if applicable)
  2. Court documents proving abandonment (if applicable)
  3. Documentation from a social worker or shelter confirming your situation (if applicable)

Where do I submit the Vulnerable Child Declaration Form?

The completed form should be attached to your main NSFAS application package.

You can submit your application online or deliver it physically to an NSFAS service center.
