List of Documents Required to Claim UIF

  1. UI-19 Form: This form is completed by your employer. It details your employment history and the reason for termination of service. Ensure it’s filled out accurately.

  3. Banking Details: Proof of your banking details, usually in the form of a bank statement not older than three months. This is where your UIF payments will be deposited.
